If you try everything listed in the replies and it still doesn't work, it's probably
dead. IME, modems fail for no obvious reason and if it's a software modem, the failure
rate is even higher.. Try a different modem if you have one available, or buy a
decent hardware modem. (I prefer US Robotics/3Com, they seem to last longer over
all) In all my years of building computers, I've only had one software modem work
on par with a hardware modem, and it lasted less than a year before failing. Good
