It appears you have a header or footer set in your user config.
You should blank out that dialog box and save your settings.

Your device driver for that item is trashed.
Find the disk that came with your video card (or motherboard).
If you don't have that, go to the manufacturer's site
and download the video device driver for your model.

In Device Manager, Remove that device (Display Adapter).
When you restart Windoze, it will prompt you to replace the missing driver.
Point it at the install media.

When you get things working properly,
back up all your device drivers to external media (all on a single disk).

That sounds like a completely seperate issue
(though it *would* be device driver-related).
If it is a USB device, follow the directions that came with the unit;
different devices want to do it different ways.
Again: Remove the existing driver if you are trying to reinstall.

...and update your malware killer(s) and scan for infections.
