If it is a computer in your home, I'd suspect an infection.
From
Safe Mode, update your malware killer(s) and run a scan.
If the computer is sometimes out of your control,
I suspect someone used System
Policy Editor.
If the computer is corporate property, I would suspect the latter
and would think you should have been alerted to changes.
