Maximising a window hides Title Bar

Hi there,

When I maximise any window in xp the title bar is hidden. If I double click the top 
of the window, it reappears. 

How can I restore my system so that when a window is maximised, the title bar with 
minimise/maximise/close buttons are always shown?
